Letters can be requested for students needing special accommodations for achievement tests such as the SAT, ACT, LSAT, etc. In order to base the letter on current vision data, the patient must have a vision exam by Dr. Jacobs within a year or less from the time of the request. These detailed letters must conform to very specific requirements by the testing service and involve significant preparation time. Therefore, we require 30 days notice for these documents. There will be a minimum fee of $150 depending on the complexity of the document produced.
